Would you like to say thank you to someone who has made a difference? Maybe it's a carer, GP, bus driver, supermarket worker, family member or volunteer - whoever it is, we would love to hear about them.

To mark the 72nd anniversary of the NHS, we're taking a moment to thank the nation for efforts to respond to the pandemic and we would like to hear from you.

Share your personal thank you to shine a light on someone who has made a difference to you or your family - and we will be featuring your messages on our social media channels and website.

It is quick and easy to get involved. Choose one of three ways to share your message…1) a short video or 2) a photo and message or 3) a handwritten note (take a photo of the message). Remember to include who you are thanking and why they made a difference to you.

Post your thanks to Twitter or Facebook, including the campaign hashtag #SWLThankYouTogetherand tag @SWLNHS by Friday 3 July to be featured on our website and thank you film.
