Live-out Nanny Add-ons

hello! I am trying to estimate the actual cost of having a live-out nanny. 
So far I understand most nannies will only agree to net rate per hour, want paid bank holiday days on top of vacation days (of course this must go over the statutory minimum days), food provided during work hours, a kitty for activities. Do we need to pay for transport as well? Is there anything else I should consider?

Pension costs.
We've always agreed a net rate per hour and then worked out the gross rate and specified that in the contract and then the payroll company works out the monthly net cost. The tax/NI bill comes quarterly so is usually an unpleasant shock!
We pay for transport when on the job (eg taking the children somewhere on the train). 
Our nanny is also insured on our car which was an extra cost. 
All our nannies have been very sensible about spending on food/trips/activities.
