I am currently tutoring a boy with similar issues and we have been making great progress during the Summer hols and throughout homeschooling.
Little and often may be the way forward and if he is a good reader sounds like it may be more Dyscalculia, confusion around maths. Sounds like he could do with a confidence boost too

I have also completed the Montessori Dyslexia training and my view is optimistic, in that, dyslexic or not, given the right support children are just as capable of making excellent progress all round : )
