My husband does odd household jobs and garden tidying/turfing etc. and I do agree that, if he arrives with the client at a given time, then the "meter runs" from that time. Getting toolsfrom a vehicle into the working area and leaving the place cleared and tidy before leaving on time is also part of his paid time. If a given timescale is quoted, then it's only fair to allow him to leave as he often has another client to go on to. If he ran late with the morning job (as people often ask for "just one more thing") then the client should a) respect that this may not be possible without his risking being late for another client (not a good reputation to have), and b) if he is able to stay longer then the client should be prepared to pay for his time. If he ran late with every "can you just..." client, it would not be fair on the next client. As for paying for someone's time, it is not ethical to ask anyone to work longer and not be remunerated for their time and effort. My husband works alone, stops for a few minutes to have a bite to eat at lunchtime (not an hour, like some) and always welcomes a cuppa if offered but works like a Trojan!
